Menstrual irregularities refer to deviations from the normal pattern of the menstrual cycle. While a typical menstrual cycle lasts between 21 and 35 days, this range can vary from woman to woman.

These irregularities can affect various aspects of menstruation, including cycle regularity, flow intensity, duration of bleeding, associated pain, or even the complete absence of menstruation.

Different types of irregularities can occur within one cycle: early periods, delayed periods, alternating early and late periods, and intermenstrual bleeding. Regarding flow and duration, there may be heavy bleeding with normal duration, or light periods, where either the duration or quantity of bleeding is reduced. Also included are excessive and prolonged periods as well as recurrent, irregular intermenstrual bleeding that occurs outside the regular menstrual cycle.

From the perspective of Traditional Chinese Medicine (TCM), the etiology of menstrual irregularities includes a range of factors such as overwork, emotional stress, excessive physical exertion, exposure to cold, congenital weakness of Shen (Kidneys), frequent childbirth within short time spans, surgical procedures, and even the use of hormonal birth control like the pill or intrauterine devices (IUDs). These factors may result in disharmony between Yin and Yang, Qi and Blood, and among the organs Shen (Kidney), Pi (Spleen), Gan (Liver), as well as the meridians Ren Mai and Chong Mai, which regulate the uterus and menstrual flow.

A detailed analysis of the cycle pattern, intensity and duration of bleeding, color and consistency of the blood, and the presence of clots is essential. This is followed by a precise differentiation between deficiency and excess, cold or heat, and identifying which organ or meridian system is involved. Only then can Chinese herbal therapy, acupuncture, dietary recommendations, and lifestyle guidance be specifically tailored to effectively regulate menstrual irregularities.
